Social prescribing: Live Well Navigators

Our Live Well Navigators can work with you to set goals and create a personal plan which will encourage and support you to:

  • self-manage long-term health conditions
  • improve your general wellbeing 
  • enjoy greater participation in community life and positive activities

What you can expect

A Navigator will work alongside you on a one-to-one basis for up to eight sessions, tailoring your health and wellbeing journey to you. They will listen to you and explore your strengths, aspirations, barriers and motivations. 

Each referral is assessed for suitability. Once your referral is allocated to the service, one of our Live Well Navigators will be in touch to arrange your first appointment. 

Your Navigator will meet you at home or in a community setting, wherever is best for you. 

Together, you will set goals and create a personal plan so that you can improve your wellbeing, building on your interests and encouraging wider participation in community life.

Eligibility

Live Well Navigators are able to work with individuals who:

  • have one or more long-term health condition(s)
  • are registered with a Swindon GP
  • are not currently working with a Community Psychiatric Nurse or Mental Health Recovery Worker
  • are not on the sex offenders register
  • are aged 18 or older
