Hypertension health coaching
If you have been diagnosed with high blood pressure (hypertension), our free Hypertension Health Coaching programme can support you to make positive changes to your health and wellbeing.
Whether you want to stop smoking, lose weight, be more active or feel healthier, a trained health coach will work with you at your own pace. Together, you can make realistic lifestyle changes that fit your daily life and are easier to maintain long term.
How health coaching can help
High blood pressure often has no symptoms. Over time, it can increase your risk of serious health conditions such as heart attack, stroke and kidney disease.
Making healthier lifestyle choices can help you:
- lower your blood pressure
- reduce your risk of heart disease and stroke
- achieve and maintain a healthier weight
- become more physically active
- improve your diet
- stop smoking
- improve your sleep
- manage stress and improve your wellbeing
- feel more confident managing your health
For some people, these improvements may also help reduce the amount of blood pressure medication they need. Any changes to medication will be reviewed by your GP or healthcare professional.
What to expect
You will be matched with a dedicated Health Improvement Coach and offered up to six one-to-one coaching sessions.
Sessions can take place in your local community or another convenient location.
Your coach will help you to:
- focus on what matters most to you
- set realistic and achievable goals
- build healthy habits that last
- identify barriers and find practical solutions
- recognise and celebrate your progress
Our coaching is centred around your goals, your lifestyle and the changes that are important to you. The support is designed to help you make lasting improvements to your health beyond the end of your coaching sessions.
Who can access the programme?
You may be able to access the programme if you:
- are aged 18 or over
- live in Swindon
- have been diagnosed with high blood pressure (hypertension)
We particularly encourage you to take part if you smoke or would like support to improve your diet, increase your physical activity or achieve a healthier weight.
