The Live Well Swindon Hub’s main role is an information, advice and triage service managing all referrals and enquiries coming into the Community Health and Wellbeing Team. This could be referrals from GP’s, GWH, or other local agencies such as Lift Psychology and Mind, as well as self-referrals.
The Hub triage system looks at the complete needs of the individual rather than addressing single behaviours, focusing on the client journey.
The Hub also oversees all the bookings for both the Stop Smoking Service and the free NHS Health Check service.

As first point of contact the Hub has one phone number and one email address and can be contacted between 9.00am and 5.00pm from Monday – Thursday and on Friday's between 9.00am and 4.30pm.
The Hub will answer any questions about referrals. You can also call to ask about any other services we offer, to talk to a specific team member or to book and/or rearrange appointments.
