Health coaching

Our Health Coaches will listen and provide advice and guidance on how to improve your health and wellbeing. This will include setting and monitoring goals, signposting to activities and using digital apps to support your health journey.

What you can expect

You may be looking to eat healthier, move more or increase your energy levels. Whatever your goal, your Health Coach will tailor your journey to your individual needs.

Each referral is assessed for suitability. Once your referral is allocated to the service, one of our Live Well Health Coaches will be in touch to arrange your first session. 

Your coaching sessions can be anywhere you feel at ease, whether it is a video call, phone call, or face-to-face meeting in a community setting like a library, community centre, or even a relaxing park walk.

This one-to-one support can last up to six sessions. Sessions will focus on supporting you to gain the knowledge, skills and confidence to take charge of your own health. 

Eligibility

The Health Coach offer is designed to work with individuals who:

  • are motivated and ready to make a positive behaviour change, and actively participate in the programme
  • are receiving means-tested benefits or in a low-income household
  • live in Swindon
  • are aged 18 or older
