How to make a referral

Referrals to OK4U are made through Swindon Borough Council Adult Social Care for adults aged 18 or over whose primary health condition is a learning disability. You cannot refer yourself to OK4U.

You can be referred by:

your existing social worker
contacting Swindon Borough Council and asking for Adult Social Care on 01793 445500
visiting www.swindon.gov.uk

Please note: OK4U is a chargeable service and is not free. Support is subject to a Care Act assessment and financial assessment, and you may be required to pay a contribution towards your care.
