Use this form to report a bright light disturbance.
To complete this form and have your case assigned to an investigating officer, you must upload completed diary sheets and statements for a period of three weeks. You can still complete this form without uploading documents, but your case will remain ‘pending’ until you have uploaded the relevant documents. If you haven't done this within six weeks of completing the form, we will automatically close your report. We will send you an email after three weeks, reminding you to upload your diary documents.
