Social prescribing: Live Well Navigators
Live Well Navigators support you to improve your health and wellbeing. They work with you to set goals and take small, practical steps that fit your life.
What a Live Well Navigator can help with
A navigator works with you to:
- manage long-term health conditions
- improve your overall wellbeing
- take part in activities in your community
What to expect
You will work one to one with a Live Well Navigator for up to eight sessions.
Your navigator will:
- listen to what matters to you
- help you understand your strengths and goals
- support you to overcome any barriers
- help you stay motivated
Together, you will create a personal plan based on your interests. This may include finding local activities or services that support your wellbeing.
You can meet your navigator at home or in a community setting, depending on what works best for you.
We will assess your referral to make sure the service is right for you. Once your referral is allocated, a navigator will contact you to arrange your first appointment.

Who can use this service
You can be referred if you:
- have one or more long-term health conditions
- are registered with a Swindon GP
- are aged 18 or over
- are not currently working with a Community Psychiatric Nurse or Mental Health Recovery Worker
- are not on the sex offenders register
