Steps to health exercise referral

Our Steps to Health Exercise Referral programme promotes better health through physical activity. It is designed to provide client focused support for up to 12 weeks for individuals who require more structured assistance in the management or prevention of many health conditions.

The Steps to Health programme is designed for the following individuals who:

  • have been referred to us by a Health Care Professional
  • are aged 18 or over and a resident in the Borough of Swindon or registered with a GP within the Swindon Locality
  • have at least one risk factor of, or is living with, a stable long term health condition amenable to Physical Activity
  • is clinically stable, so have had no events or change in symptoms or medication during the previous month
  • are inactive/sedentary and ready to make a change to their physical activity levels
  • are Independently mobile (with or without an aid)
  • are able to communicate, participate and self-monitor and do not require 1:1 supervision

The Steps to Health programme offers a free initial consultation followed by:

  • a choice of group-based activities, such as gentle circuits or gentle walking (£3.00 per session, after a free initial session)
  • further encouragement and support to be active outside of these sessions
  • signposting to other services
