Preparing for Adulthood Protocol 2025-2029

The Preparing for Adulthood Protocol 2025-2029 (PDF) has been co-produced with young people, parents and carers and wider colleagues and partners within the SEND Local Area Partnership, such as BSW ICB and Adult Social Care.

The protocol is designed to be interactive with links to relevant pages to give more information on certain topics.

The protocol will be reviewed annually to stay current and will feed back progress transparently via the SEND Partnership. Progress will be measured against clear PfA outcomes across all the four pillars included in the protocol.

Transitions for children and young people with SEND

A virtual booklet, SW Transitions regional project 2023, has been produced following work carried out by the Regional Parent Carer Forums, and completed for NHS England South West on transitions.

It is intended for professionals and families, along with children and young people, when supporting those going through transition.
