Report a substance use related incident

Swindon and Wiltshire’s Public Health teams co-ordinate intelligence gathering for suspected adulterated drug supply via its local drug information system reporting process.

Your information is vital in helping to prevent drug-related harm and deaths in Swindon. If you suspect substances are adulterated, stronger than expected, or if you have experienced or witnessed a severe or unexpected adverse reaction to a substance, please report it immediately through the Swindon Local Drug Information System (LDIS) below.

Reporting quickly and confidentially enables our public health team and partners to assess the risk and issue local drug alerts to protect others. By sharing your concern, you are contributing directly to the safety of our community.

Members of the public, family members, those who use substances, or professionals, can report concerns directly and confidentially by using the form below.

Please read the Local Drug Information System Privacy Notice before you complete the form.
