Service summary

Eastern Outreach offer outreach support for learners with SEND and or SEMH needs who are unable to or waiting to attend mainstream or special schools. Learners engage in learning activities either in the home or in local community spaces with regular outings to parks, libraries, museums, woodland and beaches.

Learners work towards individual outcomes directly linked to their EHCP targets as well as literacy and numeracy, social and emotional, wellbeing and communication outcomes needed to help them move to the next part of their journey. This could be transitioning to a special school, transitioning into mainstream or moving onto a post-16 provision like College, Supported Internship or Apprenticeship.

The approach focuses on providing bespoke, learner-centred services, ensuring that each young person receives the support they need to thrive academically, socially, and emotionally:

  • Successful Transitions to Mainstream and Post-16 Pathways
  • Achievement of Recognised Qualifications
  • Supported Work Experience Placements

Eastern Outreach services are delivered through a combination of:

  • live online lessons: Interactive and engaging sessions delivered by experienced educators, covering core subjects and personal development areas.
  • 1:1 and Small Group Support: Tailored support addressing individual learning needs, delivered virtually and through outreach.
  • trauma Informed Wellbeing Support: Pastoral care and mentoring to support emotional well-being, resilience, and social development.
  • collaborative Partnerships: Working closely with families, schools, and external agencies to ensure a joined-up approach to education and care.
