Bridge to Adulthood team - preparing for adulthood

The move from childhood to adulthood is an important time for young people aged 13 to 25. It is a time of change, new opportunities and decisions about the future, such as careers, education, social life and where to live.

Planning ahead, and having clear information, advice and support, can help young people achieve their goals.

Young people with disabilities, additional needs or who are vulnerable may need extra help during this time. For those with identified needs at school, a process called 'transition planning' starts in year 9 to make sure the right support is in place.

If a young person aged 13 or over may need adult social care services when they turn 18, an online referral form should be completed. This helps adult social care to start working with them early to plan and prepare for adulthood.
