Common assessment framework

Swindon’s children, families and community health service has launched the Early Help Record and Plan (EHRP) which replaces the Common Assessment Form (CAF)

This early help guidance has been produced to give all practitioners a useful set of tools to support single agency and multi-disciplinary practice. It includes a common pathway for assessment, planning and review, including Special Educational Needs and Disability (SEND).
