Special Provision in Swindon – SEN Continuum

This booklet is designed for parents/carers, head teachers, teachers, and staff in schools and agencies in the Borough.  It is a summary of the in-Borough continuum of provision available for children and young people with special educational needs (SEN).  The booklet contains descriptions of the current admission criteria for each Special Resourced Provision (SRP) in a mainstream school and the special schools.  This will enable parents, professionals and pupils to make informed choices about provision in the Borough.

Transitions Protocol

The Transitions Protocol has been created for successful outcomes led transition process for all Swindon young people with additional needs (aged 13 to 25), providing them with a robust and clear pathway of support.

The Transitions Protocol  and associated documents are held on the Local Offer website .

Transitions Referral Form and Consent Form

The Transitions/Learning Disability Referral Form and Consent Form should be completed for young people, age 14+, who are likely to transition to a service from Adult Social Care when they reach 18 years old. This is to ensure that Adult Social Care can start to work with the young person from an early age, to plan and prepare for adulthood.
