Portage for children with disabilities

This service, we refer to as 'Swindon Portage', provides an educational home visiting service for children who meet the criteria shown below. The service is provided in conjunction with, and linking to, other professionals and agencies.

We support the child into pre-school and school and act as the 'Lead Professional' if the child's family wish us to, usually in the form of weekly visits to your home. We aim to make the weekly visit fun and to support your child to take the next steps in their development. We design individual plans to meet the needs of both your child and your family. This includes ideas from other therapists and professionals that may be involved.

Who the service is for

This service is for children up to the age of 4 years and 11 months who have:

  • special educational needs or learning difficulties, and/or
  • physical or mental disabilities
Please note: Children that attend a local authority nursery or school, or who receive four or more sessions in a pre-school setting are not eligible for this specific service.   

Who can request this service

Children may be referred to this service by their parents/guardians or, by referral from another childcare practitioner, professional or agency.

Next steps

When your Request for Service application has been received, we will contact you to advise you whether or not your child has been put on the waiting list. We aim to do this within 14 days.

When a space becomes available, one of the staff will visit you for four to five weeks to provide some activities whilst on the waiting list.
