Amusement Licence - Section 34 5E

Apply/renew a permit for premises that provide amusements with prizes where the maximum stake is 30p and the maximum prize £25.



    Animal Boarding Licence

    Licence for animal boarding establishments required under The Animal Boarding Establishments Act 1963.



      Club Premises Certificate

      Use this form to apply for a Club Premises Licence for supply of alchol, regulated entertainment and provision of late night refreshment under the Licensing Act 2003.



        Club Premises Declaration for Certificate

        Use this form to apply for a Club Premises Licence for supply of alcohol, regulated entertainment and provision of late night refreshment under the Licensing Act 2003.



          Club Premises Vary Licence

          This form enables the holder of an an existing Club Premises Licence to make any changes to the original licence, including activities and permitted hours.



            Cultivation Licence

            You will need to complete this form if you would like to place plants and flowers, etc alongside the public highway.



              Dangerous Wild Animals Licence

              To apply for a licence to keep any dangerous wild animals under the Dangerous Wild Animals Act 1976.



                Dog Breeding Business Licence

                Application for a licence to keep a breeding establishment for dogs under the Breeding Dogs Act 1973.



                  Event (Temporary) Notice and Licence

                  To apply for a temporary activity as described in the Licensing Act 2003.



                    Explosives (Bulk Storage) Licence

                    Application for registration of premises to store explosives.



                      Fireworks Sales and Supply Licence

                      To apply for a special ‘Year Round Licence’ to sell fireworks.



                        Lotteries/Amusements Licence

                        The Lotteries and Amusements Act controls the conduct of small lotteries, raffles etc. by societies raising money for charitable, sports and other similar purposes. Organisations wishing to raise money in this way must use this form to register their details.



                          Overhang Licence

                          You will need to complete this form if any part of your premises, including signage etc, overhangs the highway.



                            Personal Licence

                            All sales/supplies of alcohol must be made by, or under the authority of, a Personal Licence Holder. Use this form to apply for a Personal Licence.



                              Personal Licence - Disclosure of convictions and declaration

                              To be used in conjunction with a Personal Licence. Please note if you apply online for your Personal Licence, this declaration form will be inluded durign your application.



                                Pet Shop Licence

                                Use this form to apply for a licence to sell animals, as pets, to members of the public.



                                  Petroleum Licence

                                  Application for a Licence (or renewal) within the terms of the Petroleum (Consolidation) Act 1928 or transfer within the terms of the (Petroleum) (Transfer of Licences) Act 1936



                                    Poisons - Licence to Sell

                                    Application by a person to have his name entered in a local authority’s list of persons entitled to sell non-medicinal poisons included in Part II of the Poisons List.



                                      Premises Licence

                                      Use this form to apply for a Premises Licence for the supply of alcohol, regulated entertainment and provision of late night refreshment under the Licensing Act 2003.



                                        Premises Licence - Transfer

                                        Use this form to apply to transfer a Premises Licence. You must ensure that you also have the declaration from the existing holder of the licence, agreeing to part with the licence.



                                          Premises Licence - Consent of existing licencee

                                          Application for transfer of an existing Premises Licence, including consent of existing licence holder.



                                            Premises Licence - Consent of Individual being specified as Supervisor

                                            Use this form to give your consent to be designated a Premises Supervisor under the Licensing Act 2003.



                                              Premises Licence - Interim Authority Notice

                                              This is an interim notice to enable a premises to continue operating under the Licensing Act 2003 (e.g. if a Premises Licence holder dies).



                                                Premises Licence - Notification of Interest

                                                If you have a land interest in a property (e.g. a freehold) you can ask for active notification of any changes to a licence that applies to it under the Licensing Act 2003.



                                                  Premises Licence - Provisional Statement

                                                  A Provisional Statement is an alternative to applying for a Premises Licence directly, when new premises are being built or existing premises are being altered dramatically.



                                                    Premises Licence - Vary

                                                    Use this form to vary an existing Premises Licence under the Licensing Act 2003.



                                                      Premises Licence - Vary and Specify Supervisor

                                                      Use this form to designate a Premises Supervisor under the Licensing Act 2003. Please ensure that your nominated person has signed the official form, agreeing to be the designated premises supervisor.



                                                        Riding Establishment Licence

                                                        To apply for a licence to keep horses for the purpose of their being let out on hire for riding or for the purpose of their being used in providing instruction for riding in return for payment.



                                                          Scrap Metal/Breakers Yard Licence

                                                          The dealing in scrap metal or the running of a scrap metal business is controlled by the Scrap Metal Dealers Act 1964. Use this form to apply for a licence.



                                                            Skin-Piercing/Tattooing Registration

                                                            Persons who commercially carry out certain skin peircing activities (include acupuncture, tattooing, ear peircing, etc.) require a licence. Use this form to apply.



                                                              Street Collecting Licence

                                                              Use this for to apply for permission to collect money or sell articles in streets or public places within Swindon for the benefit of charitable or other purposes.



                                                                Taxi Licensing - Knowledge Test

                                                                Use this form to book a Hackney Carriage or Private Hire dirver's knowledge test.



                                                                  Track Betting Licence

                                                                  At a track or sports ground, this licence allows the proprietor to permit bookmakers to trade on the premises. Use this form to apply.
