Intermediate Care - Project Review

The 'Strategic Business Review of Intermediate Care and its links with Admission Avoidance project was commissioned in May 2004 by the Director of Operations at the Swindon PCT.

The aim of this project was to review existing intermediate care services that provide support in- and out-of-hours, with the aim of strengthening care pathways and systems, avoiding unnecessary admission, securing early discharge and optimising present service provision. Where appropriate proposed are alternative service models.
