Integration Projects

Senior officers within Swindon Primary Care Trust and the local authority agreed that integration work between health and social care could be more fruitfully advanced, and agreed for an officer to be seconded to work on these areas.  

Specific areas were initially identified by the Director of Modernisation and Partnerships for initial PCT agreement and then discussed with the Assistant Director for Social Care within Swindon Borough Council’s Social Services Department. Further suggestions and recommendations arising from this meeting were then included within the scoping details.

The themes are supportive of key areas identified in the PCT’s Clinical Governance Plan, Local Authority Social Service business improvement plan (May 2004 covering a nine-month period through 2004), the recent inspection from the Department of Health of Older Person’s services November 2003 and identified work streams for Adult Care Services in particular.
