The completed Assessment Questionnaire will be transmitted to our Care Line Team, where the staff are very knowledgeable about the range of social care services provided by Swindon Borough Council and other agencies in the area.They will look at the information you have given, and decide how best we can help you. Depending on the type of problems you have told us about, they may contact you themselves, or arrange for one of our specialists to contact you (this might be a nurse, or an occupational therapist, or a social worker).
Regardless of who is responding to you, you can expect a response within two working days of you completing the online Assessment form. If you don’t hear from us in that time, please contact Care Line (see below):
After that, what happens depends very much on your circumstances.
Council-funded Adult Social Care are restricted by cash limits and must be targeted at the most vulnerable people. If your current needs don’t put you at a substantial risk, then it may be that the service we can offer you will have to be limited to information, and sign-posting you to other agencies who may be able to help you
If you do have needs that are eligible for help from the Council, and these needs are straightforward (such as needs that can be met by providing a simple piece of equipment, or providing Community Meals for a short time) then it may be that all that is needed is a single contact with you to agree that’s all that’s required, and the service can be arranged.
If your needs are more complex or long-term, then it’s likely that a fuller assessment is needed by one of our specialist practitioners, involving a personal discussion to decide the best package of care to suit your needs, and on-going support to make sure that the package is working for you.
