Asking for Help from Adult Social Care
If you think you need help from Adult Social Care, then you can use our online Assessment Questionnaire (please see below). This enables you to tell us about any difficulties you are having in managing to live safely and independently at home, and what you think may help.
You can also use it to tell us about concerns you might have about someone you know, such as a member of your family, and you wish to contact Adult Social Care on their behalf.

Who are the services for?
They are for adults (over 18 years of age) who live in the Borough of Swindon.
Anyone else?
Yes. Adult Social Care has responsibility for recognising and supporting all carers, including young carers of adults.
Another Way to Contact Us
If you don't want to complete the online questionnaire, then you can instead telephone Care Line and discuss the problem with them. Please see: Care Line.
What Happens Next?
The completed form will be sent to our Care Line Team, who are knowledgeable about the range of social care services provided by Swindon Borough Council and other local agencies.
The Care Line Team will look at the information you have given and the type of difficulties you have told us about. You will have a response within two working days.
It depends upon your specific needs whether services from Adult Social Care will be offered and the type of service (e.g. information, equipment, care or support).
In all cases we aim to be as helpful as possible, so that you have the information and support to enable you to get the most suitable help.
