Applying for a CitizenCard

Applying for a CitizenCard

What you need to enable you to apply:

1. An Application Form

2. Consent

3. A Referee

4. Two passport-size photographs

5. Payment

Sending Your Application

Application forms, photographs and any payments required must be returned to the CitizenCard Card Centre to be processed. The postal address is:

Please Note:

CitizenCard is registered under the Data Protection Act and guarantee that they will not pass on any of the cardholder’s details to any other organisation or body. To view their full Data Protection Statement log on to the CitizenCard website (www.citizencard.com).

Warning:

Every single application form is verified. Providing false information is a criminal offence.

CitizenCard Ltd. will always prosecute anyone attempting to obtain a card by deception.
