How to access services:

• Request information or an assessment yourself by phoning Careline on 0800 0856666.
• Request help through your GP or other health or social care professional with whom you are already in contact.
• To gain access as a carer to services through Adult Social Care, we advise you to have a carer's assessment.
• To enable Adult Social Care to decide whether the person you care for is eligible for services, an assessment of their needs will be offered to them.

These assessments are an opportunity to discuss your circumstances and needs and look at possible solutions and support options.
