The listings in this directory include details of how services can be accessed. Often people can approach the service direct. However, other services require an assessment to be made first, to decide eligibility and determine whether there will be a charge, and what it will be.
Referral
Self referral
In this directory ‘Self Referral’ means people are welcome to contact the organisation or service direct. Sometimes it may be the person’s relative, friend or carer who makes contact on their behalf. Others use the support of an advocate.
Increasingly health services accept referrals directly for assessment, but it still may be helpful to pinpoint the help you need with your GP first.

Referral via GP/JCT
GPs are the first point of contact to access most specialist health services, including hospital outpatient and inpatient care. See health care. GPs may also refer people to non-health services such as social services or housing, if it helps them maintain independence or well-being. See explanation of Joint Community team (JCT) here.
