Intermediate and Preventative Care

Intermediate care is the name given to the range of services which bridge the gap between home and hospital, preventing unnecessary hospital admissions or helping the transition back to home and to independence. These services may be delivered in people's own homes, or elsewhere in the community. They often invole some form of rehabilitation.

Betjeman Centre (The Great Western Hospital)
A day hospital facility providing nursing, occupational therapy and physiotherapy services for the elderly.

British Red Cross - Home from Hospital Service (The Great Western Hospital))
Practical help and support for people living alone shortly after discharge from hospital.

Community Nursing - Evening and Night Service (Primary Care Trust)
Out-of-hours district nursing services in the evening, night-time or weekends.

Community Physiotherapy Service (Primary Care Trust) (Community Rehabilitation Team)
Offers assessment and treatment for older people and adults with disabilities.

Community Rehabilitation Team (CRT) (Primary Care Trust, Swindon Borough Council and Swindon and Marlborough NHS Trust)
An inter-disciplinary team providing rehabilitation to people living in Swindon.

Community Stroke Team (CST) (Primary Care Trust)
Early discharge team for patients who have had a stroke and who will require further rehabilitation at home.

Continence Advisory Service (Primary Care Trust)
Assessments, advice and provision of disposable incontinence products to people in their own homes.

Crisis Support Service (CSS) (Swindon Borough Council and Primary Care Trust)
Short-term support to people over the age of 65 who are facing a medical crisis.

District Nursing Team (Primary Care Trust and Swindon Borough Council)
Provides nursing services to all with a health or social care need registered with a GP in Swindon.

Joint Community Teams (JCTs) (Swindon Borough Council and Primary Care Trust)
Health and social care provided in the community and delivered by district nurses, social workers and occupational therapists.

Learning Disabilities - Community Nursing Service (Primary Care Trust)
Offers support to individuals of any age who require specialist help with learning disabilities.

Marie Curie Nursing
Offers practical nursing care to people with terminal cancer in their own homes.

Mental Health Outreach Service (Avon and Wiltshire Mental Health Partnership)
Works intensively with people who are reluctant to engage with mental health services.

Montgomery Suite
An NHS service for people with high dependency and some rehabilitation needs.

NHS Walk in Centre
Offers a range of services and advice on general health issues.

Nutrition and Dietetic Service (Primary Care Trust)
Assessment and advice on all matters relating to diet and health.

Patient Advice and Liaison Service (PALS)
Provides information and support to people who use NHS services in any way, eg: patients, carers and relatives.

Physiotherapy-Learning Disabilities Service (Primary Care Trust)
Assessment and treatment for people with learning disabilities.

Prospect Hospice
Palliative care for terminally-ill people with cancer, motor neurone disease or AIDS.

Rehabilitation Units (Swindon Borough Council and Primary Care Trust)
Provide older people with a programme of residential recuperation or rehabilitation.
