People leaving hospital after treatment often need special help and support while they recouperate and rehabilitate - either at home, or if they are particularly frail or unsteady in a rehabilitation or intermediate care centre. This section lists people and services on hand in the Swindon area to help make the transition from hospital back to the community as smooth as possible.

Betjeman Centre (The Great Western Hospital)
A day hospital facility providing nursing, occupational therapy and physiotherapy services for the elderly.

British Red Cross - Home from Hospital Service (The Great Western Hospital))
Practical help and support for people living alone shortly after discharge from hospital.

Community Nursing - Evening and Night Service (Primary Care Trust)
Out-of-hours district nursing services in the evening, night-time or weekends.

Provide older people with a programme of residential recuperation or rehabilitation.

Swindon Intermediate Care Centre (SwICC), (Primary Care Trust)
A 60-bed rehabilitation centre on the same site as TGW, staffed by specialist rehabilitation teams.
